Meet

Jacob Evans, MD, is a board-certified comprehensive ophthalmologist providing medical and surgical eye care. He completed his undergraduate degree in Neuroscience at Brigham Young University and earned his medical degree from the University of Iowa Carver College of Medicine, where he was inducted into the Alpha Omega Alpha Honor Medical Society and received distinctions in both research and teaching. Dr. Evans completed his ophthalmology residency at the University of Wisconsin–Madison, contributing research to ocular medical journals and presenting at national ophthalmology meetings.

Dr. Evans cares for a wide range of eye conditions which include:

• Cataract evaluations and surgery

• LASIK and refractive procedures

• Glaucoma lasers and surgery

• Ocular injections for diabetic eye disease and macular degeneration

• Corneal surface procedures

• Eyelid and brow procedures

• Botox

• Specialized dry eye evaluation and treatment

He is known for his approachable, educational style and takes time to help patients understand their diagnoses and treatment options. He often uses pictures, models, and simple explanations to make complex information easier to follow.

Originally from Utah, Dr. Evans enjoys mountain biking, pickleball, running, and spending time with his children. He is a member of the American Academy of Ophthalmology and the American Society of Cataract and Refractive Surgeons, and he is now accepting new patients.
